As a courtesy to our clients, Sparkleen Dental does accept payment directly from a patient's insurance company for a patient's dental care when the following conditions are met and understood:
We are happy to assist our patients with filling in a dental claim insurance form so they can contact their employer or dental insurance company directly to claim their dental benefits.
Please note that not all of our services may be covered by your insurance carrier and that every insurance plan has unique conditions and exceptions. Our office will do our best to assist our patients in understanding the details of their insurance plans. However, we are limited in the information we can access. It is ultimately the patient's responsibility to understand their insurance policy and contact their insurance to clarify what is covered in their plan.
Unless prior arrangements have been made, payment is due upon completion of treatment. We accept Interac, Cash, e-Transfer, Mastercard, Visa and American Express.
If there are any outstanding charges on a patient's account, payment is expected in full before their next appointment. If full payment is not made before a patient's next appointment, their appointment may be rescheduled until their outstanding amount is paid in full.
